コロナワクチン、ファイザーのコミナティについて、名古屋の美容皮膚科医が解説
こんにちは、Wスキンクリニック名古屋、美容皮膚科医の加藤晃司です。
今回は、ファイザーのコミナティ（Comirnaty）について説明します。
私は毎年、コロナワクチンとインフルエンザワクチンを打っていますが、今年からWスキンクリニック名古屋でも、コロナワクチン、インフルエンザワクチンの接種を開始することにしました。
- コミナティ（Comirnaty）とは？
コミナティは、ファイザー（Pfizer）とバイオンテック（BioNTech）が共同開発した、新型コロナウイルス（COVID-19）の予防ワクチンです。正式名称は「BNT162b2」で、mRNA（メッセンジャーRNA）技術を利用して作られたワクチンです。2020年12月に緊急使用許可が出され、世界各国で接種が開始されました。
- ワクチンの種類：mRNAワクチン
コミナティはmRNAワクチンと呼ばれ、従来のワクチンとは異なる技術を使用しています。mRNAワクチンは、ウイルスそのものを使用せず、ウイルスのスパイクタンパク質の遺伝情報を細胞に送り込み、体内で一時的にそのタンパク質を作らせ、免疫反応を誘導するものです。
mRNAワクチンの仕組み
メッセンジャーRNA（mRNA）が注射によって体内に入る。
体内の細胞がmRNAの指示に従い、ウイルスのスパイクタンパク質（新型コロナウイルスの外側にある突起部分）を一時的に生成。
免疫システムがそのスパイクタンパク質を異物として認識し、抗体を作り出す。
抗体が生成されることで、将来的に新型コロナウイルスに感染した際、スパイクタンパク質に対して免疫応答が迅速に働き、感染を防ぐか、重症化を抑える。
- 有効性
コミナティは、臨床試験において約95%の有効性が報告されました。これにより、新型コロナウイルス感染症を予防する効果が非常に高いことが確認されています。
感染予防効果：ワクチン接種後、体内で抗体が作られ、感染そのものを予防します。
重症化予防：感染した場合でも、ワクチン接種者は重症化するリスクが大幅に減少します。
変異株への効果：デルタ株、オミクロン株などの変異株に対しても一定の効果が確認されていますが、特にオミクロン株に対してはブースター（追加）接種による免疫強化が推奨されています。
- 接種スケジュール
ファイザーのコミナティは、通常2回接種が必要です。
初回接種（1回目）：0日目に接種。
2回目接種：1回目から21日後に接種（3週間後）。
その後、変異株に対する防御効果を高めるために、**ブースター接種（3回目）**が推奨されています。ブースターは、2回目接種から6か月以上経過した後に行われます。
- 副作用
コミナティ接種後に報告されている副作用は、一般的には軽度または中程度のもので、多くの場合、数日以内に消失します。代表的な副作用には以下のようなものがあります。
注射部位の痛み（最も一般的）
疲労感
頭痛
筋肉痛
寒気
発熱
関節痛
注射部位の腫れや赤み
また、稀に報告される重篤な副作用として、アナフィラキシーショック（重いアレルギー反応）や心筋炎・心膜炎がありますが、これらは非常にまれです。
- ブースター接種の必要性
変異株（特にオミクロン株）の流行により、時間の経過とともにワクチンの効果が低下することが確認されました。このため、ブースター接種（3回目、4回目）が推奨され、免疫を強化する目的で接種されています。ブースター接種により、変異株に対する防御効果が高まり、感染予防および重症化予防効果が維持されます。
- 特殊な場合の接種（妊婦・授乳中・免疫不全の方など）
妊婦や授乳中の方：ファイザーのワクチンは妊娠中や授乳中の女性に安全であると考えられており、感染リスクを減らすために推奨されています。
免疫不全の方：免疫力が低下している方（例：癌治療中の患者や免疫抑制療法を受けている方）は、ワクチンを接種しても十分な免疫がつかないことがあります。そのため、3回目の接種が早めに行われることが推奨されています。
- ワクチンの保管と取り扱い
ファイザーのコミナティワクチンは、特殊な温度管理が必要です。
保管温度：ワクチンは超低温（-70℃）で保管される必要があります。通常の冷凍庫や冷蔵庫では保管できないため、流通や接種の際に厳しい温度管理が行われます。
解凍後の使用期限：ワクチンが解凍された後は、通常の冷蔵庫の温度で5日間保存できます。
まとめ
ファイザーのコミナティワクチンは、COVID-19に対して非常に高い予防効果を発揮するmRNAワクチンです。2回の接種で感染と重症化のリスクを大幅に減少させることができ、ブースター接種で変異株に対する効果を高めることができます。副作用は一般的に軽度で、短期間で解消されることが多いです。
新型コロナウイルス感染症から自分自身や周囲の人を守るために、適切なワクチン接種を行うことが重要です。

COVID-19 Vaccine: Explanation of Pfizer’s Comirnaty by a Cosmetic Dermatologist in Nagoya

I receive both the COVID-19 and influenza vaccines every year, and from this year, W Skin Clinic Nagoya will also start offering both vaccines.
- What is Comirnaty (Pfizer COVID-19 Vaccine)?
Comirnaty is a COVID-19 vaccine jointly developed by Pfizer and BioNTech. Its formal name is "BNT162b2," and it utilizes mRNA (messenger RNA) technology. The vaccine was authorized for emergency use in December 2020 and has been widely administered globally.
- Vaccine Type: mRNA Vaccine
Comirnaty is an mRNA vaccine, which works differently from traditional vaccines. Instead of using the virus itself, it delivers the genetic information for the virus's spike protein to the body’s cells. The cells then temporarily produce this protein to trigger an immune response.
How mRNA Vaccines Work:
The mRNA enters the body through the injection.
The body’s cells follow the mRNA’s instructions and produce the spike protein (found on the surface of the COVID-19 virus).
The immune system recognizes the spike protein as foreign and produces antibodies.
These antibodies protect the body, allowing a faster immune response if exposed to the actual virus, preventing infection or reducing severe symptoms.
- Effectiveness
Clinical trials have shown Comirnaty to have an efficacy rate of about 95%, demonstrating its high effectiveness in preventing COVID-19.
Infection Prevention: After vaccination, antibodies are produced to prevent infection.
Severe Disease Prevention: Even if infection occurs, vaccinated individuals are at a significantly lower risk of developing severe symptoms.
Effectiveness Against Variants: While effective against variants like the Delta and Omicron strains, booster doses are recommended to maintain strong immunity, particularly against Omicron.
- Vaccination Schedule
Comirnaty typically requires two doses:
1st Dose: Administered on day 0.
2nd Dose: Given 21 days after the first dose (3 weeks later).
A booster dose (3rd shot) is recommended at least 6 months after the second dose to enhance protection against variants.
- Side Effects
Most side effects from the Comirnaty vaccine are mild or moderate, often resolving within a few days. Common side effects include:
Pain at the injection site (most common)
Fatigue
Headache
Muscle pain
Chills
Fever
Joint pain
Swelling or redness at the injection site
Severe side effects, such as anaphylaxis (a serious allergic reaction) and myocarditis or pericarditis, are rare but have been reported.
- Booster Shots
As variants like Omicron have become more prevalent, studies have shown that vaccine efficacy decreases over time. Therefore, booster doses (3rd or 4th) are recommended to strengthen immunity and maintain protection against infection and severe disease caused by variants.
- Vaccination in Special Cases (Pregnant Women, Immunocompromised Individuals, etc.)
Pregnant or Breastfeeding Women: The Pfizer vaccine is considered safe for pregnant and breastfeeding women and is recommended to reduce the risk of infection.
Immunocompromised Individuals: People with weakened immune systems (e.g., cancer patients or those undergoing immunosuppressive therapy) may not develop a strong immune response after vaccination. In such cases, an early 3rd dose is often recommended.
- Storage and Handling of the Vaccine
Pfizer’s Comirnaty requires strict temperature control.
Storage Temperature: The vaccine must be stored at ultra-low temperatures (-70°C). It cannot be stored in standard freezers or refrigerators, so special measures are in place for storage and transportation.
Post-Thaw Shelf Life: After thawing, the vaccine can be stored in a standard refrigerator for up to 5 days.
Conclusion
Pfizer’s Comirnaty is a highly effective mRNA vaccine for preventing COVID-19. It significantly reduces the risk of infection and severe disease after two doses, and booster shots enhance its effectiveness against variants. Side effects are generally mild and short-lived.
